We do know Hideo Kojima planned for Metal Gear Solid V: The Phantom Pain to be the most mature game in the series. This description tells us the game has high violence.

The fact that Metal Gear Solid V: The Phantom Pain has “motiveless killing” and “violence towards defenceless people” means players have the freedom to do whatever they want. You can go up to no good like a GTA game from the sounds of things.

Metal Gear Solid V: The Phantom Pain could be like Metal Gear Solid 3. In that game, there were some scared scientists that you could beat up or kill. It’s likely similar defenseless characters will be running around the levels in The Phantom Pain.
